New Patient Registration

If you wish to register with this practice you will need to come into the surgery and put in your request with the receptionist.

You will be provided with a form to fill in some details:        

  • Name and address
  • Date of birth
  • Your former address if different
  • Your current doctor
  • Any repeat medication you take
  • The reason you are changing doctors

You must also bring in proof of address i.e. utility bill with your name and address on.

Once you have completed this form you should hand it into the receptionist who will tell you to contact the surgery the day after, either by telephone or in person.

If the doctor agrees to accept you on to his/her practice list you will need to complete an official form (GMS1) which will be sent to the Health Authority so that your medical records can be requested from you previous doctor.

You will also be given a letter of welcome, a questionnaire, a copy of the practice leaflet and an appointment to see one of the nurses for a brief medical check. This is because it can take a while for us to receive your medical records, particularly if you have come from another town.
